Full service detailsPick by the situation, not the diagnosis.
If you're unsure which service fits, our dispatch team will ask a few simple questions and recommend the right level — no upsell, no pressure.
"They can walk on their own."
Level 1 — Ambulatory. Sedan or minivan, curb-to-curb with optional walk-in assistance. Most affordable, fastest to dispatch.
"They use a wheelchair or need an arm."
Level 2 — Wheelchair & Assisted. Wheelchair van with door-through-door support and trained transfer help.
"They can't sit up."
Level 3 — Stretcher. Two-person crew, hospital-grade stretcher, bedside-to-bedside transfer. The right answer when sitting isn't safe.
"Other companies said no."
Level 4 — Bariatric. Equipment rated to 750 lbs, trained handling. We take the patients others turn away.
"It's not a medical appointment."
Level 5 — Custom. Wedding, reunion, grocery run, multi-city transfer — anything that doesn't fit the standard medical-ride template.
"It's complicated — multiple needs."
Call dispatch. We routinely combine levels (e.g., stretcher pickup + bariatric handling + long-distance) and quote one flat rate.
The details that set Astra apart.
Recurring schedule lock-in
Set up a permanent schedule (e.g., M-W-F dialysis) once. We hold your pickup window for as long as you need it, with the same crew when possible.
Hospital discharge coordination
We work with case managers, charge nurses, and bedside teams to schedule discharges efficiently — including same-day requests.
Wait-and-return service
For shorter appointments, our driver waits with the vehicle and returns the client home — no scheduling a second trip, no waiting in the lobby alone.
Long-distance medical travel
Non-emergency transfers between cities for specialty care — including MD Anderson referrals, San Antonio rehab facilities, and Dallas specialty hospitals.
Bedside-to-bedside support
Stretcher clients are transferred from their bed at home or facility, secured in the vehicle, and transferred to their destination bed — no in-between gaps.
Family communication updates
Optional text or call updates to a designated family member at pickup, arrival, and return — peace of mind without the constant phone-checking.
Clear pricing. No surprise charges.
Most NEMT pricing is built to confuse. Ours is built to be quoted upfront and matched to the invoice — without "fuel surcharges," hidden minimums, or surprise wait-time fees.
Private Pay
Flat-rate by mobility level and distance. Quoted before booking. Payable by card, ACH, or invoice.
