Level 5 · Custom Care

Because life happens outside the doctor's office, too.

Long-distance medical transfers to MD Anderson. A grandfather's wheelchair-accessible ride to his granddaughter's wedding. Multi-stop trips that include the pharmacy, the bank, and the salon. The grocery run that's been impossible since the stroke. Level 5 is for everything that doesn't fit into a standard medical-ride template — because life doesn't, either.

Why This Service Exists

Mobility shouldn't shrink your life.

A wheelchair, walker, or stretcher need shouldn't mean missing your granddaughter's wedding. A 250-mile transfer to a specialty hospital shouldn't require a medical-grade ambulance and a $4,000 bill. A trip to the salon, the grocery store, or your sister's birthday shouldn't be impossible. Level 5 was built for the things every other service forgets — because they're not "medical enough" to count. They count to you. They count to us.

What's Included

Wherever you need to be, however you need to travel.

Long-distance transfers across Texas

Houston ↔ Dallas, Austin, San Antonio, Galveston, Beaumont, College Station — we run multi-hour transfers regularly, with proper rest stops and crew rotation when needed.

Specialty hospital transfers

MD Anderson referrals, Methodist Texas Medical Center, Memorial Hermann transfers, Baylor specialty appointments — coordinated with your clinical team.

Weddings & family events

Grandparents shouldn't miss the wedding. Reunions matter. Religious services matter. Our wheelchair-accessible vehicles and trained crews make it possible.

Errands & multi-stop trips

Pharmacy, bank, grocery store, hairdresser, lunch with a friend — all in one outing. The kind of trip a family member would make happen, except now we do.

Out-of-network rehab placements

Discharge to a specialty rehab facility outside the standard Medicaid radius? We coordinate with case management and quote the transfer at a flat rate.

Flat-rate, transparent pricing

Every Level 5 trip is quoted as a single flat rate — no per-mile surprises, no "fuel surcharges," no time-based gotchas. What we quote is what you pay.

Common Use Cases

Real reasons real Houston families book Level 5.

"Mom hasn't been to a salon in two years."

Wheelchair-accessible transport for a haircut and pedicure, with the driver helping inside. A small thing — and a huge thing.

"Grandpa has to make it to the wedding."

Roundtrip wheelchair-accessible transport from the assisted living facility to the church and reception, with our crew on standby.

"We're transferring dad to MD Anderson."

Stretcher transport from a Houston-area hospital to MD Anderson in the Medical Center, or to specialty hospitals in Dallas/Austin/San Antonio.

"He just needs to run errands."

Multi-stop outing: pharmacy pickup, bank, grocery store, lunch with the wife. Driver waits between stops at standard wait-time rates.

"She wants to spend a weekend with her sister in Beaumont."

Long-distance one-way drop-off, or roundtrip with a flexible return. We coordinate the schedule, the equipment, and the route.

"Religious services every Sunday morning."

Recurring wheelchair-accessible transport to church, mosque, temple — whatever the weekly practice is. Set the schedule once.

A Real Houston Family

"He made it to the wedding. That's all that mattered."

"My grandfather hadn't left his nursing home in 18 months. He has Parkinson's and a wheelchair, and the family had basically accepted he wasn't going to make it to my wedding. My mom called Astra on a whim. They quoted a flat rate for the roundtrip, sent a wheelchair van with two crew members, and Grandpa was at the ceremony, the photos, AND the first hour of the reception. He was glowing the entire time. I'll never forget that they made it happen."

Honest Answers

Questions about custom and long-distance trips.

"How far will you actually drive?"
Anywhere in Texas, regularly. Common runs: Houston ↔ Dallas, Austin, San Antonio, Galveston, Beaumont, College Station, Tyler, Corpus Christi. Out-of-state is possible with advance planning — call dispatch for a quote. For runs over four hours, we schedule rest stops and may rotate crews on stretcher transports.
"How does pricing work for non-medical trips?"
Flat-rate by trip, not per-mile metered. We quote based on distance, equipment level (sedan vs wheelchair van vs stretcher), wait time, and number of stops. The quote is the price — no surcharges. Most non-medical wheelchair trips in Greater Houston run $85–$175 depending on duration and stops.
"Does insurance ever cover this?"
For non-medical trips, no — insurance covers medically necessary transport only. For long-distance medical transfers (specialty hospital to home, inter-facility moves), Medicaid and some Medicare Advantage plans do cover transport when medically appropriate. We'll verify before booking.
"Can the driver wait between stops?"
Yes — for multi-stop outings (pharmacy, lunch, salon), the driver waits with the vehicle and helps at each stop. Wait time is included in the flat quote up to a reasonable limit. Longer waits (e.g., during a wedding ceremony) are quoted separately.
"What if my loved one needs medical equipment along the way?"
Our vehicles are equipped to handle portable oxygen, IV poles, feeding pumps, and similar equipment. For longer transfers we also coordinate medication timing with the family or care team. If there's something specific you need, mention it during booking.
"What about overnight or multi-day trips?"
For multi-day stays (e.g., a weekend with family out of town), the typical arrangement is one-way drop-off, then a separately scheduled return. Some clients prefer roundtrip with the driver staying overnight — possible, just call dispatch and we'll quote.
"Can I book this for someone who isn't elderly or in a wheelchair?"
Yes. Level 5 is the right service for any non-medical transport need where you want a professional, dignified, insured driver — not a rideshare. We've taken people to job interviews, court appearances, immigration appointments, and family funerals. If you need a ride and want to do it right, call us.
